• OCPD background check (submitted by City staff)
• Photo ID
• Immigration affidavit (if applicable)
• Zoning verification (completed by City staff)

Required Documents for annual renewal

• OSBI background check
• OCPD background check (submitted by City staff)
• Photo ID
• General application (if not completed online)
• Immigration affidavit (if applicable or not on file)

*Required documents must be submitted to license staff for OCPD background check, may take 3-5 business days.
